Our chiropractic patients in San Diego regularly bring up this issue and ask about prevention. And why shouldn&#8217;t you?  Some news coverage has demonized the sun as unsafe. It has sent many running from it like Vampires at dawn.  </p>
<p>The truth is that overexposure to the sun can cause problems.  What you probably don&#8217;t know &#8211; underexposure can be dangerous to your health. Here&#8217;s why: A new study looking at the relationship between vitamin D serum levels and the risk of colon and breast cancer across the globe has estimated the number of cases of cancer that could be prevented each year if vitamin D3 levels met the target proposed by researchers.</p>
<p>Cedric F. Garland, Dr.P.H., is a cancer prevention specialist at the Moores Cancer Center at the University of California, San Diego (UCSD). Colleagues estimate that 250,000 cases of colorectal cancer and 350,000 cases of breast cancer could be prevented worldwide by increasing intake of vitamin D3. This is particular in countries north of the equator. Vitamin D3 is available through diet, supplements and exposure of the skin to direct sunlight.</p>
<p>The research paper which looks at the dose-response relationship between vitamin D and cancer will be published in the August edition of the journal of Nutrition Reviews. It stated that for the first time, we are saying that 600,000 cases of breast and colorectal cancer may possibly be prevented each year worldwide, including nearly 150,000 in the United States alone,&#8221; said study co-author Garland. </p>
<p>The study combined data from surveys of serum vitamin D levels during winter from 15 countries. It is the first such study to look at satellite measurements of sunshine and cloud cover in countries where actual blood serum levels of vitamin D3 had also been determined. The data was then applied to 177 countries to estimate the average serum level of a vitamin D metabolite of people living there.</p>
<p>The data revealed an inverse association of serum vitamin D with risk of colorectal and breast cancer. The protective effect began at levels ranging from 24 to 32 nanograms per milliliter of 25-hydroxyvitamin D concentration in the serum. The 25-hydroxyvitamin D level is the main indicator of vitamin D status. The late winter average 25-hydroxyvitamin D in the U.S. is about 15-18 ng/ml. The researchers maintain that increasing vitamin D levels in populations, particularly those in northern climates, has the potential to both prevent and possibly serve as an adjunct to existing treatments for cancer.</p>
<p>It was stated that the suitable dose of vitamin D in order to reach this level could be very small in a lifeguard in Southern California. On the opposite end of the spectrum, it can be quite a lot for someone in Northern Europe who tends to remain indoors most of the year. This could be best achieved with a combination of diet, supplements and short intervals of 10 or 15 minutes a day in the sun. It could be less for very fair-skinned individuals. </p>
<p>The serum level recommended by the study would correspond to intake of 2000 International Units per day of vitamin D3 for a meaningful reduction in colorectal cancer. The researchers recommend 2000 IU/day, plus, when weather allows, a few minutes in the sun with at least 40% of the skin exposed, for a meaningful reduction in breast cancer incidence, unless the individual has a history of skin cancer or a photosensitivity disease.  Garland also recommends moderate sun exposure and use of clothing and a hat when in the sun longer than 15 minutes.</p>

The key to treatment of skin cancers is prevention and early detection.</p>
<p>Skin cancer treatment will depend on several factors. The type of tumor, size, location, previous treatments, and patient factors all play into the type of treatment which must be undertaken. For melanoma tumors, aggressive treatment is necessary and well defined through national standards. For basal cell and squamous cell skin cancers, treatment is more reliant on the location of the tumor, size, and patient preference. Cryotherapy (freezing the cancer), topical medications which burn the tumor, and surgical excision are all options which should be discussed with the patient.</p>
<p>If surgical excision is the treatment of choice, for basal cell and squamous cell carcinomas in sensitive areas such as the face, eyes, ears, nose, and lips, Mohs micrographic surgery may be necessary. Mohs surgery is named after Dr. Frederic Mohs, an early 20th century surgeon who developed a special technique of examining pathologic skin specimens. The procedures allows for preservation of as much normal tissue as possible.</p>
<p>An ounce of prevention is worth a pound of cure with skin cancer. Use of appropriate sunscreens, excess sun avoidance, and tanning avoidance are good ways to help prevent skin cancers from forming. These tumors don&#8217;t generally appear overnight and are the result of a lifetime&#8217;s worth of sun exposure and sometimes sun abuse.</p>

<p>Most people already know that they should wear sunscreen when they go outside. However, few people actually apply sunscreen every time they leave the house. Usually, we only think about putting on sunscreen when we expect to be outside all day. We&#8217;ve all heard the warnings to wear sunscreen and avoid too much sun exposure. Unfortunately, young people tend to disregard these warnings and don&#8217;t think seriously about skin cancer.</p>
<p>No matter how young or old you are, you need to take good care of your skin. Excessive exposure to the sun will lead to damage eventually even if it doesn&#8217;t appear for many years. Therefore, you should prevent damage from the sun when you are young so that it won&#8217;t become an issue when you&#8217;re older. Many steps can be taken to limit or prevent sun damage.</p>
<p>We often only consider applying sunscreen when we expect to be outside for many hours. However, we don&#8217;t consider it when we spend shorter periods of time out of doors. Your skin is exposed to the sun&#8217;s rays every time you are outside, even if you&#8217;re simply walking to work in the morning or taking a breather outdoors during the afternoon.</p>
<p>In general, you should protect yourself from the sun by applying sunblock with an SPF of 15 or more on all exposed skin. In addition, the sun is strongest between 10 AM and 3 PM. Therefore, you shouldn&#8217;t go outside unnecessarily during those hours. The summer isn&#8217;t the only time you should watch your exposure to the sun. In the winter and fall, you can still get too much sunlight. Sunscreen should be worn all year round.</p>
<p>If you sweat or go swimming, you should reapply your suntan lotion every so often. To be safe, apply more lotion every two hours, or more often for children. Infants less than six months old should not be exposed to the sun any more than necessary, since their skin cannot handle sunscreen.</p>
<p>Keep babies covered with clothing and hats to limit their exposure. The delicate skin of children and babies is very susceptible to damage and should be protected especially carefully. Babies are very vulnerable to skin cancer and the sun.</p>

Warn older children that prolonged unprotected exposure to the sun&#8217;s harmful rays can eventually lead to skin cancer and that they would be safer in the shade.</p>
<p>There are several ways that you can detect the possibility of skin cancer on your own time. Keep an eye out for any changes on your skin or growth in moles or the sudden appearance of new ones. Be careful of those that seem to have increased in size or are shaped irregularly. In addition, watch out for transparent or multicolored growths. Lastly, it&#8217;s helpful to have your doctor check out any wounds or cuts that seem to take longer than usual to heal.</p>
<p>Make a routine check of your skin a regular part of your doctor&#8217;s visit. You should make sure you examine your skin every month. This is one of the best ways to prevent skin cancer. You are your own best advocate for your health. Make sure you keep an eye on your skin and your health and get treatment immediately if you develop a problem.</p>
